As a PhD graduate, can I opt for a 2 year PSW instead of 3?

No.

You can leave the UK or switch status at any point during the 3 years, but there is no option to apply for less time upfront.

Thanks. If I leave do I get IHS refund? If I switch will it be prorated and applied to the next visa category I switch to? Hope this makes sense.

No.

You need to pay again, but if your new visa overlaps your old one by more than 6 months, you will get a partial refund on the basis of having paid twice.
