HSMP-JR - Documents about Earnings - Self Employed

Hi.
I will apply for ILR under HSMP JR.

Could you please advice me what type of documents I should prepare?

I am Self-Employed, so no P60s, etc.

Does my accountant need to prepare Balance Sheet as well as Profit and Loss?
What type of Letter shall I take from my Accountant? Does anybody has a template?

I will also give my Business Bank Statements as a proof of my earnings.

PLEASE ADVICE me what else shall I prepare?
