tier 1 Form question? Part D - plz help!!!!

Hello there!

One of my friends is applying for the tier-1 extn and just stuck at the section-D ( immigration history ) was wondering if someone can help with that please.

She entered uk intially on WHM in jan2004 but just before her visa was due to expire, she went back to her home country and applied for students permit and got it and came back to uk before the WHM expiered. so now when they are asking questions (D2, D3 and D4) in the Tier-1 form that where did applicant obtain EC/Visa before travelling to UK etc etc?!!.... wht should she mention? Did she mention jan 04 (when she orignally came to the UK) or dec 05 ( when she entered the uk on student permit) or when she got her last tier1 permit??

please help!!

That would be her last entry clearance to The UK. The new visa she got will be labelled as a entry clearance visa.
