NCS client care form

I will be grateful if anyone could explain what the client care forms are, used, for. I have only now noticed that there is an error in this.

My wife, my minor daughter and I are applying for BC after 5 years plus work permit and 1 year ILR. We submitted our marriage certificate, which was copied and attested and included in the paperwork in support of the application. However, the client care says that no marriage certificate was available. Could this affect our applications?

I will be grateful for any advice.
