Can GP/ NHS be requested for a letter of number of visits ?

I am collecting documents for cohabitation. Can I request

1. NHS or GP to provide the dates & number of visits of my wife to GP and
2. the date she got registered at GP ?

Your doctor/ GP will give you a standard letter writing that you and your spouse living together more than two years. It worked mine.

you can ask similar type of letter from your council.
