Proof for name change

Hi,

I have changed my name recently (3 years back). My degree certificate still carries the old name. As per the law in India, I have got court affidavit to get my name changed officially. All my new documents like passport , driving license are in my new name.

What type of proof should I provide to HO for HSMP processing?

Harsha, based upon what you posted .... the Court Affidavit.

u can also attach newspaper AD with affidavit,,,only if u gave that time...
