PSW Application Form Help Required

I am filling up my PSW application form and wondering if some one could guide on me few things. Following are the things which are confusing me a bit.

B10: Home Office Reference Number:

I inserted the reference number i found on Home Office letter head they sent me with my last visa. I am wondering if that is right thing to do?

C6: Current Passport or Travel Document:

I put in my passport number, nationality, issue and expiry date, place of issue. I am wondering if i need to put in my ICFN number as well? I read the guide lines and it states, provide passport, travel document or ICFN. It makes me think if they have asked for only 1 item or more?

C9: Current Identity Card for Foreign Nationals (ICFN):

Is it the pink card which they are now giving out to students for their visa renewals?

D4: Enter The Referenc number on the entry clearance or visa which can be found in the applicants passport or travel document:

What am i supposed to put in there? the VAF number written on my first visa which was issued at backhome or something else?

J1: Course Start and End Date:

Now, the UNI letter i got doesn't state the end date of course, instead it states the Final Examination date and Results/Awards Date. In my final Examination date, it just shows the month and year, no mentioning of day. I am wondering if i should put in my Result/Award date?
