PSW DEPENDANT TO TIER1 GEN. DEPENDANT

Hi Friends
My wife got PSW dependant visa in Oct 10, and presently she is living with me in Uk. I applied my Tier 1 General in Oct 10 before her arrival in the uk. I got my Tier1 General in Dec 10. Now i want to extend her visa as Tier 1 General dependant because she is still on PSW dep visa which is going to expire in Mar 2011.

I have following questions

1: do i have to fill up PBS Dependant (Extensions) form for her?
2: do i have to provide her proof of address ?
3: do i have to provide my tenency agreement.?

Or
Are these documents enough for her visa application ?
1: Her passport, pic , Marriage Certificate and app form
2:My 3 Bank statements
3:My Visa copy and H.O Letter

Please provide you valuable advice.

is it necessary to provide dependant's proof of address ???
