Query on Filling Tier 1(general) PBS Appendix1 form

Hi,
im applying for Tier 1 (general), i have couple of query on filling the PBS Appendix1 form.
1) on Section 4.8 Earnings Table - while filling 'Exchange Rate Applied' column, what Exchange rate should i put , how do i get it from where?

These fees are effective for all visa applications made from 06 October 2009. The consular rate of exchange is £1=Rs.80. Please note the visa fees are subject to change.

Is that the amount i should convert and put it on the above asked column?

2) i wish to make sure, is that INR salary should multiply with 5.3(as India on Band 'D') while calculating salary to equivalent Pounds,considering xchange_rate =Rs80 ( as said on vfs link)
i.e INR 440000 *5.3 =2332000/80 = £ 29150

this is to confirm whether my earnings qualified to Apply
Pls kindly confirm as im correct here with calculation

3) i dont have my name on my Birth certificate, is that 10th Mark sheet with passport or Voter identity card will be considered for confirming AGE
