Query on ILR (Mine and Dependent)

Hi,
I will be eligible for my ILR from January end since I will complete my 5 years in the UK. I have got a Tier1 (General Migrant) Visa. I have a few queries with regards to mine and my wifes application -

1. Can you please let me know if i need to fill Form O for my PR application.

2. My wife has been with me in the UK since July 2009 (1 years 6 months) and is here on a HSMP Partner Visa. Would she be eligible for PR with me and if yes would the application need to be done on the same form?

3. If she is not eligible for PR can she still continue with her HSMP Partner Visa or do I need to apply for a new visa for her? If yes, what visa and which form would it be?

Any help would be greatly appreciated. I will get back on any updates I get on this from other parties.

I got approval in May 2008 and applied for my HSMP in October 2008.

1. SET(O).
2. Your wife will not be eligible for settlement until July 2011 (319E(d)). If you do wish to wait until July 2011 to apply for settlement (self), your wife should change to spouse of settled person (FLR(M)) after you are granted settlement. She may then apply for settlement in July 2011 using SET(M) (287(a)(d)).
3. Alternatively, wait until July 2011 to apply for settlement together.
