A question on student visa

Hi

I am aware the new student visa form has been changed to PBS, what I want to know is if, there are three dependents on a student visa, do they all need a separate form when applying for an extension stay.

How I have always been doing it, is to let my spouse and 2yr son be on the same form as me, but this has now changed, plus my 21 yrold always uses FLR(o) form, does she still need to use this form or has this changed as well?
