You'll have to apply for the next extension on form FLR(M) as per the link PrestonLancs has posted.
You'll need to meet the income level of £18,600 or have £16,000 or over in savings to make up any shortfall. Joint income will be counted. 6 months bank statement + 6 months pay slips. Submit 6 documents in joint names showing your joint address, spread as evenly as possible of the 2.5 year period, from at least 3 different sources. If you don't have 6 in joint names, submit 12 in single names.
The NHS Surcharge will be £500, payable online before submitting your application.
