Referees form and support documents

Good afternoon all,
Apologies if this has been posted before.
In the process of applying for a British citizenship for my wife after just receiving her ILR. I have some queries:
1. With the two page for referee, why do we need to use passport photo with name written on the back if we’re going to be scanning the form on the UKCVAS system anyway?
2. Is that the correct form for referee to fill out? Two pages, first with name and pp photo stuck on top right and second page with referee name and date signed.
3. As my wife was dependent on me since her time here, for the last two visas (FLR and ILR), I’ve had to supply my employment, payslips, bank statements, accommodation, etc. Do I need to upload all of this again?
4. All the documents in the mandatory list are ready. Do we need to scan any extra supporting documents like children’s birth certificate, my passport scanned, utility bills, etc. Do you recommend anything?

No - there's no dependency concept for citizenship - your wife applies in her own right, and will be a British citizen in her own right.

Do we need to scan any extra supporting documents like children’s birth certificate, my passport scanned, utility bills, etc. Do you recommend anything? Bills or statements are not accepted for this, and not needed. All documents are mandatory, you don't need to add anything else, unless needed for the application. Your passport is required as she's applying as spouse of BC. Childrens' birth certs not required.
