Regarding HSMP Dependent Visa

Hi,
I am a HSMP Visa holder and currently in UK. I got my HSMP in Febraury 2005.
I wish to apply for Dependent visa for my partner. We are not married but we are living together for the past 3 years. My partner had finished her Course here in UK and currently holding a student Visa which is valid till next year.
Can anyone please tell me how do I apply for her Dependent Visa? I must get extention for the completion of 1 year of HSMP by Febraury 2006. Please advise me what documents do I have to produce and where and how to apply.
