regrading filling form

I am covered by HSMP ILR JR single applicant self employed after april and nov 2006 category.

1. In section 3 of set(o) form which option to select under section B?
2. On page 3 the first question of HSMP ILR review shud I select it or not.
3.according to rules I have to show progress of Biz and economic activity and I am planned to show following docs.
1. 12 months business accnt statements
2. accounts of biz last year
3 letter from accountant
4. invoices of last year
in 2 years between I didnt do much sales or biz didnt work as usual so didnt pay tax in these 2 yrs but never claimed any benefits
5. I have tax history of last 4 years and this years tax invoice (no tax retur because returns come in end of April)
so shud I show last 4 years history and last year tax slip?

Anything else u think I shud show ?

I have origional HSMP approval letter but dont have HSMP extension approval letter I dont know where it has gone is it necessary to show ?
