Hi,

Iam planning to apply for HSMP soon.

Due to new HR policy in my company all the employee numbers and designations got changed. Also the name in the pay slip has different names (ie., some payslips initals are expanded and some not).

Do I need to sumbit a letter from HR for the employee number and designation changes in payslips and also an affidavit for different names.

you must get a letter from employer stating all these changes. I would advise this letter should have a table containing 12 entries, one for each month, Name printed and Employer ID on salary slips. Better you write such letter and get it on company letter head.

Not really. You need two pieces of evidence to substantiate each other. So if your payslip says you got paid 1000pounds and your bank statement says the same, then I don't see an issue with the name abbreviation.
Second, you did not bother to tell us what else is on the payslip that might match up to the way it was layed out originally. I find the change of both the employee names and numbers pretty suspicioius but if your NI number is there and hasn't mysteriously changed with the rest of the payslip, then I wouldn't freak out about the changes.
