Yes indeed, you need to fill the form. Please ask if you have any question, in section 5 you need to put the net. The gross is needed in section 9 A.5.

You are fine with the documents you have. You don't need Invoice summary from the accountants. At the end of the day is your ltd company which invoice for services not your accountants (my personal opinion). Hi could you tell me why you suggest that I should apply after 24th April. If it is not earlier than 28 days and my expiry date is 27 May shouldnt I be aplying from the 30th of April.
