b. It says 'delete if not applicable' so I'd do just that (and leave the tick box empty)
c. NCS will collect the payment sheets for AN and the passport - the AN payment slip goes to the HO, the passport payment slip to HMPO - both then take payments separately.

It hopefully shouldn't be an issue for naturalisation, just explain on page 22 of the form that you have been using an anglicised version of your name. Youd be British because of your birth in the UK then, so your parents' status and whether they were married or not shouldnt matter. There are only v erylimited circumstances where you wouldnt be British, eg if your parents were enemy aliens or foreign diplomats for example. So not sure why they aske...

I think she didn't include the evidenc again, details of the evidence provided should be in the caseworker notes on their database. But who knows, if they already got the PR date wrong this may not be accurate either...

Try sending a cover letter along with a copy of your passport and DCPR to the address below and ask for the date to be corrected. The council is clearly uninformed and in the wrong - if you have PR you are entitled to social assistance on the same terms as British citizens (it even states it on the PR document).
If they continue to be difficult, contact the charity Shelter for assistance, they are really good!

There is an option on the form where you tick that you'd like your documents returned via secure delivery and then pay £3 more. All pretty useless in my case though, they just put the envelope with the passport and the one with the supporting docs through the door even though a signature was require...

If you are a Canadian citizen you do not need to take a language test. Just provide them with proof of your Canadian citizenship, I believe the guidelines say they need a passport copy or something.

It doesn't matter that your naturalisation application isn't based on your Canadian citizenship.

As I understand it, you need proof that your country of origin does not allow name changes. Somebody I know provided a letter from their Embassy for example.
