When did you pass it, in 2016? Did you take note of the reference number? Or a photo of your certificate?

No changes in the life in the UK test. Usually the official test page would say that the test is getting updated prior to the introduction of the new test. Meanwhile you can try a 2015 sample test here

1) Yes you can do this. I'm not sure how many days you will need to wait (I think it's 7 days after failing), but usually the que is 2 months in advance. I believe they Changed it in October 2013. I'm not completetely sure about it. You will need to pass both the life in the UK test and English to apply for citizenship, unless you are under 18 and one of your parents is a British Citizen.
