My question is when is she eligible for her ILR? No sooner than within 28 days before the 5th anniversary of her FLR(M) visa issue date in September 2014. You are good. As you have obtained ILR recently and used the same certificate you don't need anything new.

When applying as company director you don't need to provide any self assessment or dividend voucher. It is only required for Self-Employed ( Sole-Traders).

Also, there is no need to provide tax office address as its no more required once you provide your PAYE reference.

Make sure to send following documents when applying for passport.

1. Two passport size photo ( One photo signed)
2. Original Full birth Certificate
3. Original ILR Card (Father or Mother)
4. Original Passport (Father or Mother)

Tier 1 general does not need employment all the time. You are totally safe and can apply for ILR without any issues.
