ILR does not mean that tier 2 dependent visa is expired. I also had this belief but I called home office and got it cleared. Hi In my case I have a valid visa for my kid under my earlier Tier 2. My son is tier 2 dependent and it is valid until April 2016. I have an update.

I applied for my son (1 year old) through NCS Basingstoke on 1 April 2015 - MN1 application. The Home office have deducted the amount on 25 April 2015 for £ 669. Need to wait for the certificate to be approved and delivered.

I am also in the same boat as yours. I applied through NCS Basingstoke on 1 April 2015 for my son (1 year old) but yet to get the money deducted from the bank. Not sure when they will deduct.

NHS Maternity care in Hampshire and Wiltshire(my friend) was extra-ordinary.
So am not sure why you want to have the baby abroad.

Your child will be direct citizen of UK if at the time of birth(if baby is born in UK only), both you and your wife are settled on the date of birth.
