Thank you - much appreciated - regarding this one, it specifies the date of the release of the 'document certifying permanent residency'. Darnit - I should have checked this closer before now :( I am married to a British citizen - some sources seem to suggest this exempts you from waiting that year...

Its not a mandatory nor legal requirement to use the latest 'monster' PR form. You could go commando and apply by letter. You are not legally required to give this information for the purpose of EEA PR. If you use the form, you will need to disclose this information because the form asks for it.
