AN form - Permanen residence card - gained date vs issue dat

Hi
i would need some advice with filling in the AN form.
Basically, i have gained permanent residence on the 4th July 2016 but the AN form 2.4 only asks for issue date.
How can i include the PR gained date so that NCS and UKVI understand that i have met the 12 months requirement?

i have asked my previous employer to show my holidays in 2012 on their headed paper. So alongside their letter stating employment, P60 and my SAR request data i should be fine :-)

Thanks for the replies.

I have not used my birth certificate, but when registering for LIUK you need to state country of birth - hence me being literal and stating GDR.
Therefore, the LIUK certificate stated GDR and not Germany as on my passport.
