Could you please send me formats for Affidavit of Support and CA Statement for property. Do I need to get the CA statement on my name or it will be on my father's name.

If you haven't arranged courier for collecting the document, they will send the documents by Royal Mail. It takes 6 to 7 days. I hope you should be recieving your docuements in a day or 2. Mean while you can check with ur local post office if they have recieved it or not.

No you dont require to get your medical check up done. Once BC will take the decision on your application than they may ask for medical tests (As they are doing for couple of other permits these days)... and in that case also they will get it done thru VFS... so at this stage... no check up required ...
