The exact same thing happened with me, I filled out the form for my dependents (wife and child) and the IHS was 822 for each. My contract is 1 year so it was excessive fees. 1 no , only required for main applicant, that too only in cerain soc code jobs. 2. Not if you are applying at the same time.
