Please remove GWF0 and just type next 8 numeric on the Ref tab I too fall the in trap of typing entire reference number and found nothing. where in Bangalore you submitted?
if in Whitefield - premium centre, then sms, courier are included by default. if it is other centre, then you may have to pay to opt it during biometrics.
And if u missed it then, no chance to get it now

Ok IHS reference is fine. Just in case u can join the email printout (ihs payment acknowledgement email)

Offer letter, address proof etc are not required as CoS letter summarizes it all, however it fine just as additional proofs.
