Format/template for the countersigny's reply letter

I'm just wondering if someone have a template or a letter their counter signatory used to confirm the passport application? If so, I would appreciate if that can be attached here or PM me.

Thanks

Birth Certificates of Childrens under 18

Hi All,

Just a quick query, whether to provision children's birth certificates is mandatory at the time of FLR(HSMP), even if they have their own passports. Date of birth, place of birth and parent's name are clearly mentioned on passports.

Can anybody guide me in this regard please?

Cheers
