Looks like they are missing these documents and pages of the form. There is big mess in your application. I would contact JCAP and ask them what you should do. May be JCAP can do it again for you for free as you have already paid for these services. Worst case you will have to send your original doc...

No it will not affect in anyway. These fines just tell that you are careless when inside vehicle.
