Hmm. This is a weird one. Perhaps the 'decision made' was a mistake.

Your cousin can apply for her CAIPS notes via the CIC website to see what has been going on. These will give you details of all the activity carried out on her application by CHC.

Must admit I don't know because I didn't have to send one. Check the visa office specific instructions document - got a feeling I have seen somwhere you have to send the original. If in doubt, you could always send original and keep notarised copy.
