I have also filled Appendix2 form and the print out of online application. Not sure if you need to fill this form with the online application form or not.

i think people are getting confused with the escalations. When you send an email query, your query is assigned a case number. if they are unable to answer your query, they will escalate your query/case (not application) to bhc islamabad who will be able to provide an answer.
