If your grandmother was born in Ireland, you mother won’t have a FBR as she is automatically Irish.
The only time you need your parent’s FBR is if the Irish-born person is great-grandparent or further back.

An Irish born great grandparent only works if the applicant’s parent got FBR before the applicant’s birth. Since 1986 anyway.

Call or email your local Irish embassy - they should be able to email you a pdf of your application or issue a refund. FBR is entirely separate from the passport offices.
