I have used 600dpi to passport page that matters and 200dpi on other pages - file was just under 6MB.

I am unable to download the documents after upload - I can remove them though

"Acquired Permanent Residence" - you acquired it over a year ago so you can apply for Citizenship now.

PS. It seems if you apply online + send documents by post (old system) you get a response quicker - can anyone confirm?

Is emailed survey proof my application is on it's way?

I was sent "UK Visas and Immigration Survey" - does this mean that the application has been approved/refused and the results are on their way to do they just send it at random? Any experiences?

Proof of living in the UK

Has anyone sent just "letters from employers (including start and finish dates)" as proof of living in the UK and got accepted? Without sending Payslips, P60s etc?

When I apply online from inside the UK for a passport do I need a countersignatory?

I think I can answer my own question. In the online application there is a question Are you either: a) An EEA national who has been issued with a document certifying permanent residence b) A family member of an EEA national who has been issued with a permanent residence card Once it's answered "YES"...
