Hi, I had ILR back in 2018 and my dependent needs an extension now. So if I put in the last Cos dates, then it will be from 2015 to 2018, which is the past? BTW, I tried July 2013 to Dec 2022 and the IHS website asked me to pay £2184, which is 3.5 years instead of 3 years. I need some help on this. ...

Yep, i had an email reply from home office too.
They sent me the 07/2018 version set o paper form to take to the appoinment.
