In case if you have provided any IHS number of earlier visa and IHS calculates to 0, ignore that application and start fresh and pay correct IHS fees. This way you will reduce delay later.

You cant apply for extension. Ideally you must have applied for extension around May 2019 from UK and must have not traveled out of UK before a decision.
