As far as I’m aware the NHS surcharge isn’t required for the fiancé visa (it’s for visas longer than six months).

We didn’t include the p60 in the end.

We used the upload facility on the website, for applications within the UK you have to pay extra to have them scanned at the biometrics appointment. IDK if you have to pay extra outside the UK for this service, but personally I don't trust these people so we scanned and uploaded everything ourselve...

I wouldn't hold your breath waiting for this checklist - you should know which documents you need to send & it does state on the application form what documents you are expected to include.
