There is NO requirement that a visitor visa must only fly from country where visa was issued.

Dear gag285, 1. You must carry the appointment letters of all applicants. I was there ten days ago, and one applicant was asked to bring a printed appointment letter. They take it with your documents when your number comes for document verification and submission. 2. I am unsure about PDF documents....

I was planning that I apply for only a year's PBS Dependent visa for my child (IHS GBP 400), and subsequently, apply for British Citizenship once that visa is about to expire. This is because I would get my ILR by then (hopefully!).

Yes, you have to pay again. For instance, in my case, I had a less than three-year visa. I changed my employer in the 2nd year of my Tier-2 (General) visa. I paid again the NHS surcharge for the third year.
