In Form 9, the Following is mentioned "The original of your child’s current passport and any previous passports valid during periods of residence in the state, together with a photocopy of the biometric page of each passport." it should be filled by whoever witnessed your form.

In some applications, the application number is issued very late.
In my case, I had applied in Feb 2019 and got the application number in Aug 2019.
Don't know what was the reason?
