Three photos in total.

One for the AN form which you glued in the box on the referee page (this is clearly stated on the referee page and guide); 2 photos for the passport application.

Sorry, I didn't realise that you and your children did not receive a letter stating when you/children acquired PR.

Do check around the £ to get the verify true likeness done.

Hi silver slingshot, It's not expensive to get a commisioner of oath or a solicitor to "written confirmation of a true likeness". Have you actually asked around how much it will cost? I don't believe EEA FP can be used as a 'visitor visa'.

That's correct. Take both passport, with the valid RC in the old passport and the new passport. There is no need to transfer vignette. I travelled in the same manner for a few years. Never had a problem at border control.
