there is a section where they ask you to write anything else you feel relevant and if necessary, use a separate page to continue... i would certainly have used that or a covering letter along with it, to explain the above or past immigration history. why give them a chance to refuse the application ...

When you're sent an acknowledgment letter, HO also confirms that they received your application along with all the documents you sent with it.
