For original application, I uploaded transcript including university statement stating that course was taught in English and UK NARIC has confirmed that in their letter. I opened OCI form in Google Chrome browser.

On printing, there is no signature box on the form and they clearly mention that signature/thumb impression must be in the centre of the form.
