your accountant is correct. Also include PAYE registration letter from HMRC if you got one or ask accountant for a copy. He can provide you with that as he will have them in his records

You are not self-employed

When you are a director of LTD company and drawing a monthly salary, then you are employee and not self-employed. So the rule you quoted doesn't apply to you.
