You can't get this form anymore. The process now is once you have received a letter from HMRC with the UTR number then register for corporation tax online at HMRC website using the UTR.

A business account is not required for the initial application, best to retain your funds in your personal account until you get your visa.

End-user(s) can be a commercial or domestic customer.

Not a problem, renewal of passport is not needed. my friend got his visa yesterday. His passport had expired in January 2013. Passports are not stamped instead you will receive BRP.

UKBA only require the passport to be valid at the time of application.

Any fees paid for a service is part of operating cost, don't forget any asset you purchase or hire i.e. business pool vehicle is part and parcel.

Returning to the topic of remuneration, as long you pay the minimum wage you will be OK, whether an apprentice or older and experienced employee.

PSW period will be counted towards ILR.

After PHD you may find a course to show progression but UKBA are very unlikely to offer you anymore time to study as there is a timeline cap.

Aziz, this is not that complicated.

The operating costs in your P&L for 3 years needs to show that your outgoings (expenditure) purely for the business was £50K+.

How you kept your business afloat is not of UKBA's concern.
