I had a missing P60 for my 3rd year, company said they cannot issue me a duplicate. Then called local tax office, who sent me a letter stating how much earned and tax paid for that year. So for missing P60, you can get a letter.
Not sure what you can do about P45 though..

Hi, As far as I am aware, Employer's change of address needs to be informed to HO but not employee's address details. But you should have informed this to your local tax office so the P60 reflects your current address or the appropriate address at that time though this may not be a big issue if you ...
