In my case i did not staple the form when i attended PEO, so when i presented the form at the lady who checks and validate it, she stapled them all, when we went to make payment they just teared off the payment page.

They do require photocopies according to set M form, some people dont take copies but its recommended because Case workers seem to like it when you have copies, makes their life easier, That is if you applying at PEO.

Congratulations, i also had a fairly wonderful experience when i did my set (M) application at croydon on 3 april, i personally believe Set (M) applications are not as hard as we some times think. Set (O)s seem to be a bit more hard.

When my BRP came i was not home, the courier guy did not ask for any documents or see anything, he just asked my wifes name and asked her to sign, although she had my passport and hers to show. Some times it can depend with the courier who turns up.

I think you are on the old rules because under the new rules your visa would have been given for 3 years initially. When you got your spouse visa did they not send you a letter telling you when you can apply for ILR?

And remember that you can add an explanation in the Additional Information Page towards the back. But if you have all the weekly payslips, it will be fairly obvious that the pay fluctuates, so no problem if you average to monthly approximate. You should be done within 3 hours or so. Be patient, the waiting gets a bit much at PEO but if you have everything and remain calm and papers are organised, you will be fine. You also have evidence of living together for more than 2 years, if you meet the other requirments you could apply for the unmarried partner visa if you dont want to marry just yet, but i think that would be out of country application.
