Dependants of work permit holder can be included in the same form regardless how long they'v been in the UK.
No documents are required for the dependants other than passports and birth certificate (if any was born in the UK). It is better to attach the marriage certificate.

Yes. I asked the UKBA about this before booking the appointment.. Because no biometric record needed for this type of application (SET(o) - work permit).

I had the same case and I got ILR last week. You dont need for the limited leave to remain, just include your baby in your ILR application and attached his birth certificate to the application.
