e-Statement print out from online banking is absolutely fine. Have tried it many times and it always works. Just ensure you give the latest statements (last 6 months).

If you decide to change jobs after a year (and that is a year of reckonable residence, as proved by the GNIB stamp in your passport), the new employer would have to sponsor you for a new work permit (which either he can pay for, or you can). If you do this, it would also "reset the clock"...

Why would it be rejected? It was not a delay from your side. If not they why? You are not the only person who has this difference in dates due to the new format of the GNIB cards.

Only when you are traveling between countries boarding on plane or ferries, otherwise it is not needed if you are roaming around in cities. You will need to carry your GNIB card with you.
You do not need a visa to land in Ireland since you are an US citizen.
You do not need to carry your work permit with you. Just keep it safe, you might need it (or a copy of it) if you apply for your naturalisation a few years down the line.
