Which Form for 10 year long residence

I would like to apply for ILR under the 10 year long residence in July 2013. I not sure which form I should use SET(O) or SET(LR). Form SET(LR) require a lot documents which I need to send along e.g. Gas, electricity and water bills for each year, council tax letter for each year of my stay, doctors ...
