No need for a visa for her. Just jump on a plane and off you go. The landing in UK will be via domestic terminal so no issue at that end. The airline staff in Ireland will ask for her visa which will be the Stamp4EuFam. They may ask you to go to their desk and have the boarding card stamped as visa ...

Yes and No, Yes, For people who were originally given a Temp Stamp 4, both direct and permitted family members. Its not normally automatic thou. You gotta write to them asking for decision/extension and then they issue a letter with 2-3 months extension. You take that to Garda and they renew the car...

How best to report the incident? Do I report it to Solvit or INIS or some other department? I dont even know her name man.

Could you also please shed some light on the refusal reasons quoted. Are they not supposed to explain why they are refusing, etc?
