You have to be in this country for 5 years, so from Sep 2009 to Aug 2014.

If you first extension is upto Feb 2014, then yes you have to go for 2nd extension. NO choice

Depends where you are applying from.
For example if you are applying from India then medical document is
Mandatory showing that dependant doesn't have TB
