I applied on the 12th June 2015 using version 04/15 (old version). I got my approval last month, and have also got my naturalisation certificate.
So, hopefully it should be alright

Thanks Vinny I spoke with Home Office as well and also got a call back from Liverpool Contact Centre confirming the same. Somehow, there is a general confusion among staff in the Passport Advice Helpline about people in my situation. I have finally managed to get an appointment for the 7th of April....

I am assuming this is for ILR application. If you have unpaid leave then mention it in your cover letters. Include the start dates and end dates ( exit and entry dates for UK).As evidence, point to the stamps in your passport

for the purpose of settlement or nationality applications, UKBA can accept the IELTS certificate, irrespective of when the test was passed, provided the qualification is at B1 level.
So, no you don't have to give IELTS again.

If you read the Tier 1 dependent guidance you will find the answer there. So because you have been in the UK for 5 years, you will have to show £900 + £600 for each dependent.

You get the same points for the 'Age' as that of your initial application.
Also you might want to read on the Tier 1 Policy guidance, as to what you need to show them for previous earnings. Also try using the points calculator
